The settlement affects approximately 105,000 class members.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Hospital Authority of the City of Bainbridge and Decatur County, doing business as Memorial Hospital and Manor, agreed to settle a class action lawsuit claiming a targeted cyberattack in November 2024 led to unauthorized access to files containing sensitive personal and health information.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Who can file a claim for a data breach payout?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Class members are individuals in the United States who received notice from Memorial Hospital and Manor that the November 2024 data incident may have compromised their private information.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How much is the class action settlement payment?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Class members can claim&nbsp;<strong>one of the following<\/strong>&nbsp;cash payment options, as well as medical data monitoring:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Option A \u2013 Documented losses cash payment:<\/strong>\u00a0Class members can claim up to $5,000 in actual, documented out-of-pocket losses that occurred between Nov. 2, 2024, and Jan. 5, 2026. Losses include fees for credit monitoring, costs to replace IDs, postage to contact banks and other monetary losses due to identity theft or fraud.<\/li>\n\n\n\n<li><strong>Option B \u2013 Cash payment for lost time:<\/strong>\u00a0Class members can claim up to four hours of time spent time addressing issues related to the data incident at $25 per hour for a maximum of $100. Lost time may include changing passwords, investigating suspicious activity or researching the incident.<\/li>\n\n\n\n<li><strong>Option C \u2013 Alternate cash payment:<\/strong>\u00a0Class members who do not submit a claim for documented losses or lost time can submit a claim to receive a flat $40 cash payment.<\/li>\n\n\n\n<li><strong>Medical data monitoring:<\/strong>\u00a0All class members can elect to receive one year of CyEx Medical Shield Pro, which includes $1 million in medical identity theft insurance and monitoring for health care insurance ID exposure, medical record number exposure and unauthorized health savings account spending.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">How to claim a data breach class action rebate<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">To receive a settlement payment or free medical data monitoring, class members must submit a claim form.
